All public officials, either elected or appointed, whether paid or unpaid, shall be required to disclose any financial, business, commercial, contractual interest or other private transaction which conflicts with or otherwise adversely affects the performance of his or her official duties; such transactions shall be disclosed as a matter of public record, on the form attached to the ordinance codified herein, prescribed in § 31.026 of this chapter.
