1330.03 PERMIT.
   (a)   Permit Required. No person, firm or corporation shall demolish or commence the demolition of any building, or part of a building, without a demolition permit. A separate and distinct demolition permit is required for each building.
   (b)   Application. Application for a demolition permit shall be made to the Chief Building Inspector and such demolition permit may be issued only to the person, firm or corporation under whose direction and control of persons engaged in such demolition will be and who is directly responsible for the intended demolition. Such application shall be in writing and must contain the following: name and address of applicant, name and address of owner of the building, a scaled site map containing detailed information concerning location, construction, description and condition of the building, estimated date for commencement of demolition and estimated date of completion of demolition, method of demolition to be used, solid waste management plan specifying the intended disposition of rubble and building materials, nature, location and proof of termination of all utility connections to the building or structure, request for such use of streets, alleys and sidewalks as is anticipated, and all such other information as is determined necessary by the Chief Building Inspector in the interest of protection of the public health, safety and welfare.
   (c)   Insurance. Except as hereinafter provided, no demolition permit shall be issued until the applicant has met the insurance requirements of Section 1327.07. No proof of insurance need be furnished in regard to demolition of a one story accessory building not exceeding 600 square feet in area.
   (d)   Permit Fee.   For demolition permit fees, see Section 1335.03.
   (e)   Issuance. No demolition permit shall be issued for any building or structure in a historic district, or for any building or structure listed on the National Historic Register, until a Certificate of Appropriateness has been issued in accordance with the procedure in Chapter 1347, Historic Preservation Districts and Listed Properties of the Willoughby Codified Ordinances. This requirement may be waived if the Chief Building Inspector determines that an immediate hazard exists to the public health, safety, and welfare of the citizens.
   (f)   Valid and Effective Dates.
      (1)   Subsequent to the issuance of a demolition permit, it shall be unlawful to commence demolition work without giving the Chief Building Inspector at least 48 hours written notice of the date and time of the anticipated commencement of the demolition.
      (2)   A demolition permit shall be valid for a maximum of ninety (90) days from the date of issuance.
