Application for a license to operate a bed and breakfast establishment shall be made to the city, in writing, on a form to be furnished by the city. The application shall be verified by oath or affidavit, shall be accompanied by a non-refundable fee of $100 and shall contain the following information:
(A) The name, age, and address of the applicant;
(B) The location and description of the bed and breakfast establishment which is to be operated under the license;
(C) Whether the applicant has made similar application for a similar license or premises other than described in the application, and whether such application was granted or denied.
