The emergency response plan shall be submitted together with an affidavit on a form provided by the environmental authority of the city as to the truth and accuracy of the plan. This affidavit shall be signed under penalty of false statement by the person or, in the case of an entity, a duly authorized representative of the firm, partnership, corporation or other entity required to report hereunder.
