1468.01 FORM AND CONTENTS.

Each application for a building permit for the erection, construction, remodeling or alteration of, addition to or moving of any building shall be accompanied by a topographic survey in a form of Annex "A" incorporated herein by reference, which topographic survey shall contain the information shown on Annex "A" and be certified by a licensed engineer or surveyor.
