(a) Any person desiring a license, or a license transfer, under this chapter shall apply to the City Clerk Administrator who upon receipt shall make recommendation to the City Clerk for issuance of a license as provided in Chapter 802. Such application shall contain the names and addresses of all parties interested in the business. Such application shall further state the following:
(1) The location of the premises where a pool room or billiard room is to be located or conducted;
(2) Whether or not such pool room or billiard room is to be conducted in connection with some other kind of business, and if so, the nature of the business;
(3) The number of pool tables, billiard tables or combination pool and billiard tables in such room or place;
(4) Such other information as Council may from time to time require; and
(5) Name and address of the Resident Agent for service of process.
(b) Such application shall be signed and acknowledged before a notary public or other officer as truthful and accurate before a notary public or other officer authorized to administer oaths in the City.
(c) Such application shall be accompanied by the license fee required by Section 812.03.
