1307.05   APPLICATION FOR ISSUANCE OF A CONTRACTOR REGISTRATION.
   (a)   Any person desiring to be registered as a contractor shall apply to the Building Inspector for such registration on a form prescribed by the City Manager.
   (b)   The application for registration as a contractor shall be signed by the applicant. The application shall contain the following information:
      (1)   Name of the applicant;
      (2)   Name of the business entity to be registered by the applicant;
      (3)   Current residence and/or business address of the applicant;
      (4)   Current residence and /or business telephone number of the applicant.
   (c)   The application for contractor registration shall be accompanied by the following:
      (1)   The contractors certificate of liability insurance demonstrating the minimum bodily and property damage coverage as specified in Section 1307.07;
      (2)   Proof of registration with the Sidney Income Tax Department per Section 1307.08;
      (3)   Payment of fee per Section 1309.11.
   (d)   Upon satisfactory submission of the items required above, the Building Inspector shall issue a registration certificate. The Building Inspector may deny an application for registration if the contractor fails to submit any of the items above; if the contractor has repeatedly failed to comply with the applicable requirements of all building codes as adopted by the City or regulated by the State Building Code; or the contractor has violated the provisions of this chapter.
