The application for a permit for the sale of merchandise on the public sidewalk shall include:
(A) The name, address, and telephone number of the applicant;
(B) The name, address, and telephone number of the person who will be responsible for the conduct of the sale;
(C) The date(s) when the sale is to be conducted;
(D) The location(s) where the sale will be conducted; and
(E) Any additional information as the Planning and Building Official shall find reasonably necessary to a fair determination as to whether a permit should issue.
