§ 20-6-57 PERMIT; APPLICATION; CONTENTS.
   The application for a permit for one or more of the activities described in § 20-6-52 of this subchapter shall include:
   (A)   The name, address, and telephone number of the sponsoring organization, and if one or more of the activities will be conducted in conjunction with another organization, the name, address, and telephone number of the other organization(s);
   (B)   The name, address, and telephone number of the person who will be the chairperson responsible for the conduct of the activities;
   (C)   The date(s) when the activities are to be conducted;
   (D)   The location(s) where the activities will be conducted;
   (E)   A general description of the type(s) of activities which will be conducted on all dates applied for; and
   (F)   Any additional information as the City Council shall find reasonably necessary to a fair determination as to whether a permit should issue.
