(A) The application for a license to operate for hire a vehicle for the conveyance of passengers shall include the following:
(1) The name, age, and residence of the applicant;
(2) The applicant’s state driver’s license number;
(3) Whether the applicant has held a motor vehicle operator’s license continuously for a period of not less than two years prior to the application; and
(4) A statement to the effect that the applicant has complied with and, during the period of the license, will at all times comply with the applicable laws of the state and ordinances of the city pertaining to taxicabs and other vehicles for the conveyance of passengers.
(B) The application shall be signed by the applicant in the presence of the City Clerk. A photograph of the applicant shall be filed with the application. The City Clerk shall attach this photograph to the license when the license is issued to the applicant. At the time of filing the application, the applicant shall pay to the City Clerk the fee provided in Chapter 6, Article 6 of this code.
