1325.11   APPLICATION FOR REGISTRATION.
   Applicants for such registration shall apply at the office of the Director of Public Service and make application for registration on a form provided by the Director. Every application shall contain the name and address of the applicant, the name and address of a person who knows the whereabouts of the applicant at all times during working hours, a description of the last job performed by the applicant, a statement whether the applicant has previously engaged in contracting, construction or building operations in the City and whether the applicant has been convicted of violation of any provision of any building code and, if so, the circumstances thereof.
   If the application is for a firm, partnership, corporation or other business association, the application shall contain the names and addresses of all members or officers, as the case may be. The application shall also contain such other relevant information as the Director deems necessary.
