Every applicant for a license to operate a commercial establishment shall file a written application with the Director of Public Service containing the following information:
(a) The full name, age and residence of the applicant; if the applicant is a partnership, the full name, age and residence of each of the partners; if the applicant is a corporation, the full name of the corporation, together with the full names of its principal officers;
(b) The name under which the applicant will do business;
(c) The full name, age and residence of the person responsible for the operation, control and maintenance of the commercial operation, together with a signature of such person acknowledging his responsibility;
(d) The exact business to be conducted and the probable hours of operation;
(e) Any unusual condition connected with such commercial operation that would have a special effect upon the public health, safety or welfare.
