A. License Required; Payment Of Fee: No person, partnership, association or corporation shall establish, conduct or maintain in the city any rest home, nursing or convalescent home, or related institution without first obtaining a license and paying the tax required in section 3-1-2 of this title. The tax required shall be in addition to any fee required by the laws of the state of Oklahoma.
B. Contents Of Application; State License Required: Application for license shall be in writing showing the location of the rest home, nursing home, convalescent home or related institution, showing the size and type of structure, the number of patients or persons to be housed therein, the name of the operator of same, the number and date of the license issued by the state department of health and the type of institution or home to be conducted.
