1428.04 ENFORCEMENT.
   (a)   An application for a certificate of contractor registration shall be filed each year with the Building Department. An application may be filed no sooner than December l of the year preceding the time of registration.
   (b)   The application shall include the following:
      (1)   The name, address, business and home telephone numbers, and social security number or federal tax identification number of the applicant, type of ownership (sole proprietorship, partnership, corporation) and the responsible parties for legal notification and a copy of their driver's license or other identification approved by the Superintendent of Building and Zoning.
      (2)   The name, address and social security number of each employee and subcontractor of the applicant who are currently working on any site in the City.
      (3)   The name, address and social security number of each employee and subcontractor of the applicant who worked on any site in the City during the calendar year preceding the year of registration, provided that, in case of an applicant which files its application prior to January 31 of the year of registration, the Superintendent of Building and Zoning may issue a certificate of registration on condition that the applicant submits the information no later than January 31, and if the information is not so submitted, the Superintendent of Building and Zoning may suspend the Certificate of Registration until such time as the information is submitted, upon giving written notice to the contractor as provided in Section 1428.05(b).
   (c)   A current certificate of competency from the State of Ohio must be submitted for all plumbing, HVAC, fire protection and electrical contractors.
   (d)   A current registration must be submitted for all asbestos lead and radon abatement and testing contractors.
   (e)   No permit required by this Building and Housing Code shall be issued unless the public is protected by an approved policy of insurance, commonly termed a policy of public liability and property damage product insurance, providing for the payment of sums hereinafter specified, which policy shall be on file with the Division of Building and Zoning Inspection.
   (f)   No registration shall be issued unless the application is accompanied by an insurance policy in the following minimum amounts:
      (1)   Public liability insurance in an amount of not less than five hundred thousand dollars ($500,000.00) for injuries, including those resulting in death, of any one person, and in an amount of not less than five hundred thousand dollars ($500,000.00) on account of any one accident or occurrence.
      (2)   Property damage insurance in an amount of not less than five hundred thousand dollars ($500,000.00) for damages on account of any one accident or occurrence.
   (g)   No work shall be commenced before a registration is issued and posted on the job site. The registration shall remain posted at all times that work is in progress.
   (h)   An owner applying for a registration to work on his or her own residence shall not be required to submit the above public liability and property damage insurance policies.
