In addition to the above requirements all wells shall comply with the following:
(a) All equipment shall be muffled to minimize noise impacts on-adjacent properties.
(b) Lights shall be shielded to prevent glare onto residential properties.
(c) The driller shall install a minimum of 200' of surface casing meeting the specifications of the ODNR, Division of Oil and Gas, and shall comply with all of the provisions contained in the State report "Special Conditions for Drilling in the Lake Plain Region of Northern Lake and Ashtabula Counties". It shall be the responsibility of the driller to provide documentation to the Department of Planning and Development verifying proper placement of the surface casing.
(d) Notice shall be given to the Fire Department prior to any burn-off. Fire extinguishers shall be maintained on the site during drilling or burn-off operations.
(e) No equipment or material shall be stored on the site which is not required for the daily operation of the well.
(f) Within 60 days subsequent to the abandonment of the well, it shall be capped in conformance with state regulations and shall be visible above the surface grade. Upon satisfactory completion of said capping, the administrator shall, in writing, notify the applicant of his acceptance. The applicant shall not be considered freed of his obligations until receipt of said letter of approval from the administrator.
