It shall be the duty of each person legally authorized to perform vaccination on dogs when inoculating any dog to fill out in triplicate the certificate obtained from the city with the information required by Section 6.08.020 and immediately present one copy to the owner or harborer of the inoculated dog and to mail one copy to the office of the city clerk within five days from the date of the inoculation. The remaining copy of the certificate shall be retained by the person performing the vaccination. At the time of the inoculation of any dog, the person performing the vaccination shall also deliver to the owner or harborer of such dog the tag obtained from the city as evidence of such inoculation.
