§ 95.162 REQUIRED INFORMATION FOR COMPLAINTS.
   Any complaint of a hazard or violation under this chapter is required to contain the following information for the complaint to be processed:
   (A)   The name of the person filing the complaint;
   (B)   The address and telephone number of the person filing the complaint;
   (C)   The location of the hazard or violation; and
   (D)   The type of problem, hazard, or violation.
