(a) The registration of a registered contractor may be suspended or revoked by the review board for any of the following causes:
(1) That the registration was knowingly obtained through any fraud, misstatement, or omission of material fact;
(2) Violation of any provision of this article;
(3) The business transactions of the registered contractor are found by the review board to have been marked by a willful practice of failure to meet applicable building codes or to correct deficiencies under such codes;
(4) Violation of applicable building codes or this article;
(5) Failure or refusal to submit to alternative dispute resolution process or to honor a mediated agreement; or
(6) Violation of any provision of chapter 16 of the Code of Ordinances.
(b) The suspension or revocation shall not take place until the review board has notified the registered contractor in writing of the cause or causes for such revocation or suspension, which notice shall:
(1) Contain a brief statement of the facts relating to the suspension or revocation;
(2) Advise the registered contractor of the procedure for appeal; and
(3) Be served upon the registered contractor in person or by leaving it at the address as stated on the application for registration, or at the address on file with the review board, or by certified mail or regular first class U.S. mail addressed to the registered contractor at such address.
