Sec. 5-88. - Contents of registration application.
An application for registration shall contain, but not be limited to, the following information:
(a)   The name and residence address of each individual owner, partner, or, if a corporation, each officer thereof, and the name and residence address of the manager or person responsible for the operation of the business, if different from such individual owner, partners, or officers, and the federal identification number, if incorporated, or social security number or other taxpayer identification number of individuals;
(b)   The trade name, business address, telephone number under which the business and/or registered contractor will operate, which may include up to two (2) additional business names by which the registered contractor also operates, provided that these two (2) additional entities possess the same taxpayer identification number as the initial registrant;
(c)   A certification that the contract form which will be used when scheduling jobs or agreeing to provide services, if the work is to be performed for a member of the general public will be in compliance with the provisions of section 5-95.
(d)   Proof of liability insurance and workers' compensation insurance sufficient to demonstrate compliance with section 5-89;
(e)   A certification that the applicant will participate in, and abide by, an alternative dispute resolution process to investigate and resolve complaints regarding the applicant and will abide by any mediated agreement reached in such a process;
(f)   Identification of the person or entity, approved by the review board, which will perform mediation or other alternative dispute resolution services to investigate and resolve complaints regarding the applicant when necessary;
(g)   Proof of state or local license in good standing for trades or occupations where state or local license is required;
(h)   Certification by the applicant that it holds a current business license in Fayette County; and
(i)   Certification by the applicant that it has not been adjudicated to be in violation of any provision of chapter 16 or, if so adjudicated, the applicant has paid any fines and fully complied with any administrative orders and/or remedial measures related to the violation.
