As soon as practicable after issuance of a permit as provided for in section 5-80, the department of public works or its designated representative shall relocate or replace where necessary the monuments and shall record such monuments in accordance with the unified mapping program's system of permanent control monuments. Replacement of all monuments pursuant to this article shall meet the following requirements:
(1) Survey based upon North American datum 1929;
(2) Grid data based upon Kentucky State Plan Coordinate System; "x coordinate" shall give the position in an east direction; the other, to be known as the "y coordinate" shall give the position in a north direction;
(3) A verbal description as to location of monument;
(4) A sketch map of the new monument placement;
(5) The placement shall be a permanent monument equal to or better than the one removed; and
(6) The brass disk will be furnished by the department of public works.
