The application for a permit required by section 5-80 shall contain specific information as to the location and placement of the monument. The permit shall provide that in the event a monument is removed, disturbed or destroyed, the monument shall be resurveyed and replaced by the department of public works or its designated representative, with the reasonable administrative costs to be borne by the applicant. The permit shall also provide that, wherever it is reasonable to anticipate that a monument will have to be moved due to the proposed activities, the applicant shall promptly inform the department of public works in order that the department or its designated representative can resurvey and relocate the monument prior to the removal of the original monument. The reasonable administrative costs of such resurvey and removal shall be borne by the applicant.
