(A) Application for a storm permit shall be made to the City Civil Engineer. The application shall be in writing on a form prescribed by the City Civil Engineer.
(B) A storm water permit shall be issued if:
(1) The person is eligible to apply for and obtain a storm water permit under § 5-2-6-9;
(4) A Certificate of Sufficiency of Plan, if required, has been filed by a registered professional engaged in storm drainage design;
(5) A bond, if required by the City Civil Engineer, has been posted pursuant to § 5-2-6-17;
(6) A covenant, if required by the City Civil Engineer, has been executed pursuant to § 5-2-6-18;
(7) An easement, if required by the City Civil Engineer, has been dedicated pursuant to § 5-2-6-19;
(8) The applicable fee, computed in accordance with § 5-2-6-33, has been paid.
(9) The permit applicant has executed an agreement with the City Civil Engineer for the provision of construction inspection services in conformance with § 5-2-6-23 or, as applicable, a Certificate of Obligation to observe has been filed by a registered professional engaged in storm drainage design in conformance with § 5-2-6-11.
