1042.12   PLAN REQUIRED; APPROVALS AND CERTIFICATIONS; ISSUANCE OF PERMIT.
   The following regulations shall be complied with by a person desiring to tap into a public sanitary and/or storm sewer:
   (a)   A plan shall be filed with the Sewage Compliance Officer showing the property, the improvements thereon, the number of buildings or structures to be connected and the location of the tap in relation thereto.
   (b)   Upon the approval of the plan, the Sewage Compliance Officer shall certify his or her approval, in writing, to the Public Works Department.
   (c)   Upon the receipt of the approval from the Sewage Compliance Officer, the Public Works Department shall certify to the Department of Finance the amount of tapping fees due and payable to the Department of Finance.
   (d)   Upon the presentation of the receipt of the payment of such tapping fees from the Department of Finance, the Public Works Department shall issue the necessary permit authorizing the tap as required by Section 1042.09 .
