(a) Any applicant for a tree permit shall submit an application on a form prescribed by the village arborist as well as a plan (the "priority tree plan") that:
(1) Identifies all priority trees on the lot, parcel or tract, including their location, size (in DBH), species and condition;
(2) Indicates the nature and extent of the tree impact activity that is proposed to occur within or likely to interfere with the drip line of any priority tree; and
(3) Sets forth in detail the measures to be undertaken to protect any priority tree (but not any removable trees) on the lot, parcel or tract.
(b) The village arborist may allow the priority tree plan to be combined with any other material that may be required in connection with the activity giving rise to the tree permit requirement. In addition, the village arborist may require an applicant to submit such supplemental information that may reasonably be necessary to conduct a complete review of the application pursuant to this article.
