§ 22-41 APPLICATIONS.
   Applications for a landscaper license shall be made to the village manager on forms provided by the village and shall contain the following information:
   (a)   Name and address of the applicant; if the applicant is a partnership, the application shall show for each partner his or her name and address and interest. If the applicant is a corporation or other business entity, the application shall show the name and address of the registered agent, and each manager or director; and with respect to each shareholder or person owning at least a 5% interest in the entity or any class of its corporate shares, the person's name and address;
   (b)   The number, kind and license plate number of vehicles owned and controlled by the applicant's landscaping business;
   (c)   The location of the applicant's principal business office and garage;
   (d)   The color scheme, insignia, trade name and telephone number located upon and used to designate the vehicles of the applicant;
   (e)   A statement that the applicant has received copies of, reviewed and understands the applicable ordinances of the village and will notify the applicant's employees thereof;
   (f)   A statement that the applicant has received copies of, reviewed and understands the guide to ravines and bluffs;
   (g)   Delivery of proof of insurance with the coverage types and amounts as established from time to time by the village manager, which insurance shall name the village as an additional insured; and
   (h)   Such other information as the village manager may require from time to time.
