§ 22-36 REGISTRATION REQUIRED.
   (a)   No person shall own, establish or operate a group home in the village unless such person shall have registered with the village manager as provided in this section before operation of such home commences.
   (b)   Any person seeking to register with the village as an owner or operator of a group home shall complete a form to be provided by the village manager that includes at least the following information:
      (1)   Name, address and phone number of:
         A.   The owner of the group home;
         B.   The operator of the group home;
         C.   The individual who manages the group home; and
         D.   An individual (other than the manager) who may be contacted in the event of an emergency.
      (2)   The address and phone number of the group home;
      (3)   A brief description of the activities conducted at the group home, including the number of individuals receiving care at the group home, the nature of the care and any special needs or limitations of such individuals; and
      (4)   Evidence that the group home and its owner, operator, manager and staff have obtained and maintained all licenses, permits, certifications and approvals required by law.
   (c)   The registration required in this section shall be renewed annually or whenever any information provided in the most recent registration has changed.
   (d)   No fee shall be charged for such registration.
