(a) Application for amusements designated in section 4-43 of this chapter shall be made upon blanks to be provided by the village manager and each such application shall state:
(1) The name and address of applicant;
(2) The nature of the amusement;
(3) The place where it is proposed to conduct such amusement, if any;
(4) The nature of the apparatus or contrivance to be used therein;
(5) The price it is proposed to charge for admission to such amusement, if any;
(6) The hours during which it is proposed to operate or conduct such amusement;
(7) The time for which such license is desired; and such other information as the board may require.
(b) All such applications for a license for an amusement involving mechanical equipment or machinery shall be accompanied by a certificate of a qualified inspection agency dated not more than sixty (60) days prior to the date of application, certifying that said machinery and equipment has been inspected and found to be safe for use by the public and in good working order.
(c) All applications for any such license shall be presented to the president who shall be satisfied as to the good moral character of the applicant before any such license shall be issued to any applicant. (1963 Code § 13.902)
