The fees to be charged by the Department of Health for the following services shall be as follows:
Birth certificates | $20 per copy |
Death certificates | $25 per copy; $2.25 to Coroner's fee |
Genealogy search: Birth Death | $20 per search $25 per search |
Amendment | $20 per amendment |
Paternity affidavit | $50 to be paid by client at initial time of preparation of document |
Lab testing | $20 per order |
TST (TB) | $20 per test |
Septic permits | $600 |
Septic repair/expansion | $250 |
Septic installer registration | $50 |
Food service facility permit (# of employees) | 1 to 9 $200 10 to 20 $300 21 plus $600 |
Retail food store permit (square footage) One-half year fee applied for food service facility/retail food store per guidelines above | 1 - 1,000 $200 1,001 - 8,000 $300 Over 8,000 $600 |
Temporary/booth - food | $50 |
Annual food permit late fee | $50 per 45 days |
