(a) Each applicant seeking an initial appointment or a promotion in either the Fire or Police Departments shall file with the Mayor, by the deadline set by the Mayor, or if no deadline is set, then within a reasonable time prior to the proposed examination, a formal application in which the applicant shall state under oath or affirmation as follows:
(1) His or her full name, residence and post office address;
(2) His or her age, place and date of birth;
(3) His or her business employment and residence for the previous five years;
(4) Such other information as may reasonably be required pertaining to the applicant's fitness for the position sought; and
(5) Applicants for initial appointment in the Police Department shall provide a copy of the Ohio Peace Officer Training Council Certificate, if the applicant has such a certificate. The certificate is not required for application or appointment.
(6) Applicants for initial appointment in the Fire or Police Departments who wish to receive credits for honorable discharge from service in the uniformed services, pursuant to Section 246.09 of this chapter shall provide a copy of the DD-214 Form, or other proof of such service.
(7) Applicants for initial appointment in the Fire Department must submit proof of successful completion of the State of Ohio Firefighter Level 2 Certification Course.
(8) Applicants for initial appointment in the Fire Department must submit proof that the applicant is certified as a paramedic by the State of Ohio.
(b) Blank forms for an application shall be furnished by the Mayor to any person eligible to take the examination. All applications shall be filled with the Mayor in duplicate and one copy, receipted by the Mayor, shall be returned to the applicant.
