§ 125.19 REVOCATION; DENIAL OF RENEWAL.
   A license may be revoked, or a renewal thereof denied, for any of the following reasons:
   (A)   Abuse or neglect of a resident as defined in the Nursing Home Care Act;
   (B)   Repeated misappropriation of the property of a resident;
   (C)   The failure to maintain an activity schedule for all residents as required by the Nursing Home Care Act and regulations promulgated and authorized by the Nursing Home Care Act or the Illinois Department of Public Health’s rules and regulations referenced in § 125.03;
   (D)   Conversion of the property of a resident;
   (E)   The extreme and repeated violation of either division (J) or (K) of this section and the Nursing Home Care Act (ILCS Ch. 210, Act 45, §§ 1 et seq., including but not limited to § 3-119 of the Nursing Home Care Act (ILCS Ch. 210, Act 45, § 3-119, which is not corrected within 90 days of the first notice, not corrected thereafter within 30 days of a second notice or which cannot be corrected;
   (F)   Violation of any provision of this chapter or of the minimum standards, rules and regulations promulgated hereunder;
   (G)   The failure to provide proper security for the patient population and the general public as required under the Nursing Home Care Act;
   (H)   The failure to remediate any nuisance to the general public and adjoining property owners after 30 days’ written notice from the village;
   (I)   The failure to maintain all required village, county, state or federal licenses, permits or certificates;
   (J)   The failure to maintain proper standing with the Illinois Secretary of State for 30 days or more;
   (K)   The failure to maintain a clean, healthy and safe facility as required by law;
   (L)   The failure to provide residents with proper security to retain residents in the facility to the extent permitted by the Nursing Home Care Act;
   (M)   The failure to maintain all staff at the facility as required by law;
   (N)   Any grounds upon which an application for a license may be suspended, revoked or denied as prescribed by § 110.04 of the Village Code; and
   (O)   The failure of any licensee, owner or operator of the home to comply with the minimum standards as prescribed in this chapter or any other chapter of the Village Code.
