§ 94.20 INFORMATION REQUIRED TO OBTAIN LICENSE.
   Any person, firm, or corporation may obtain or renew a license to operate a public zoo within the village by submitting an application for license to the Village Clerk and paying the non-refundable license fee set forth in § 94.23. The application shall be on forms prescribed by the Village Administrator and shall contain the following information about the zoo:
   (A)   The name, business address and telephone number of the person, firm or corporation.
   (B)   If the zoo operator is a corporation, the corporation shall provide a certificate of good standing and the name, address and telephone number of the corporation's president and of its registered agent.
   (C)   If the applicant is a partnership, the applicant shall provide the name, addresses and phone numbers of all partners holding greater than a 5% interest.
   (D)   If the applicant is doing business under an assumed name the applicant shall provide a copy of the assumed name certificate filed with the Secretary of State of the corporation.
   (E)   The applicant shall provide proof of insurance as required in § 94.17(N) and shall agree to keep such insurance in force during the term of the license.
   (F)   The applicant shall agree to observe all provisions and conditions of this subchapter during the term of its license.
   (G)   The application shall be signed by the applicant or a responsible officer of the applicant.
   (H)   The application shall be accompanied by any zoo rules and regulations.
