721.01 DEFINITIONS.
   For purposes of this chapter, the term "amusement" or "entertainment device" means any type of shooter-type, flipper-type, kicker-type or bat-type coin-operated machine, instrument or equipment or any electronic-type game device.

721.02 LICENSE REQUIREMENT; APPLICATION.
   Any person, firm, partnership, corporation or association displaying for public patronage or keeping for operation any amusement or entertainment devices herein defined, shall be required to obtain a license therefor.
   Application for such license shall be made to the Finance Director and such application for such license shall contain the following information:
   (a)    Name, address, age, date and place of birth of the applicant.
   (b)    Address where the machine or device will be displayed or operated and business conducted at that address.
   (c)    Description, mechanical features, name of manufacturer and serial number of the machines covered by the license.

721.03 LICENSE FEE.
   Every applicant, before being granted a license, shall pay the following license fee for the privilege of maintaining or operating each such amusement device; one hundred dollars ($100.00) for the first such device; two hundred dollars ($200.00) for the second and third such device; and five hundred dollars ($500.00) for each amusement device thereafter to be placed at such address. Such license shall be an annual license. (Ord. 1980-17. Passed 4-1-80.)
721.99 PENALTY.
   Whoever violates any of the provisions herein, in addition to the revocation of such license shall be guilty of a misdemeanor of the second degree. If such offender has previously been convicted of a violation herein, any subsequent violation shall be a misdemeanor of the first degree. (Ord. 1980-17. Passed 4-l-80.)
