§ 158.16 PERMITS.
   A permit to use or occupy the highway right of way shall be obtained before an approach to a service station is constructed. The application for a permit shall be made to the City Engineer as per §§ 158.03 and 158.04. The application shall be accompanied by a detailed plan showing all of the proposed construction affecting traffic movements, both on and off the highway, and shall include the following items:
   (A)   Dimensional information relative to the exact location of the property along the highway, north point and scale of drawing.
   (B)   Property lines and the dimensions thereof.
   (C)   Width of through pavement, and if a divided highway, width of median and location of existing median crossovers.
   (D)   Distance between edge of pavement and right-of-way line.
   (E)   Width of proposed and existing drives and the radii involved.
   (F)   Dimensions and type of construction of proposed island between approaches.
   (G)   Location and dimensions of proposed buildings, pump islands and parking area.
   (H)   Location and description of existing and proposed drainage structures within highway right of way.
