(a) Every application for a rental dwelling license shall be on a form approved by the official charged with the enforcement of this Code. It shall state the name and address of the applicant, the nature and extent of his or her interest in the building for which the license is required, the name and address of the owner of the building, the location of the building, the number of dwelling units therein, the number of rooming units therein, whether the dwelling unit is to be used as a short-term rental, the number of short-term renters permitted in the dwelling unit, the property number that may be assigned by the short-term rental listing service, the name and valid telephone number of a contact person designated to receive complaints and communications 24 hours per day, every day, and such other information as may be required by the official charged with the enforcement of the Code.
(b) The rental dwelling license fees, established for the purpose of subsidizing the cost of periodic inspections, shall be as follows for all structures requiring rental dwelling licenses pursuant to Section 1360.15: one hundred dollars ($100.00) for the annual license, plus twenty-five dollars ($25.00) per dwelling unit or rooming unit, to a maximum of three thousand dollars ($3,000.00) for any structure, for structures containing four or more dwelling units or rooming units.
(c) No rental dwelling license shall be required for lawful one and two-family structures occupied by the owners as the owner's principal residence, unless they are used as short-term rentals.
