1323.19 NOTIFICATION BY PERMIT HOLDER.

   The person to whom a permit is issued shall notify the Commissioner:

   (a)   Of commencement of operations under such permit, twelve hours (12 hrs.) in advance;

   (b)   When operation is completed;

   (c)   Of locations of any borrow sites or any disposal sites for excess materials, prior to commencement of operations.
