725.02 APPLICATION; FEE; EFFECTIVE PERIOD.

   Prior to the issuance of each permit, the applicant shall furnish the make, model and serial number of the vehicle to be used and the name, address and telephone number of the owner and/or operator employed. Each applicant shall pay the sum of two dollars ($2.00) for such permit for each vehicle, which sum shall cover the period from October 15 to April 15 of each year.
