123.01 INITIATIVE, REFERENDUM AND RECALL PETITIONS.

   (a)   Methods of Obtaining. Any resident may obtain forms of petitions for the initiative, referendum and recall by making a written application to the Clerk of Council for the same.

   (b)   Receipt for Forms Obtained. Any resident securing forms of petitions for the initiative, referendum and recall shall sign a receipt stating the number of petition forms obtained.

   (c)   Limitation on Number of Forms to be Furnished. The Clerk shall not be required to furnish in excess of two hundred petition forms for the initiative, referendum or recall.

   (d)   Contents of Initiative Petitions. The Clerk is hereby authorized and directed to provide and keep in custody forms for initiative proposals, which shall read as follows:

   NOTICE

      Whoever knowingly signs this petition more than once, signs a name other than his own, or signs when not a legal voter is liable to prosecution.

   INITIATIVE PETITION

      We, the undersigned, electors of the City of Youngstown, State of Ohio, hereby propose that when three percent, or more, of all the electors of the City of Youngstown, shall have signed this petition or identical petitions of which this petition is a part, the following Ordinance to wit: (Name and No. of Ordinance) shall be submitted to the electors of the City of Youngstown, for their approval or rejection according to the procedure and in the method prescribed by the Revised Code of the State of Ohio, and in accordance with Section 82 of the Charter of the City of Youngstown, and with Section 731.41, Revised Code.

   REFERENDUM PETITION

      We, the undersigned, electors of the City of Youngstown, State of Ohio, hereby petition and when six percent, or more, of all the electors of the City of Youngstown, shall have signed this petition or identical petitions of which this petition is a part, we order, that City Ordinance No. _____ entitled "__________" be submitted to the electors of the City of Youngstown, for their approval or rejection according to the procedure and in the method prescribed by the Revised Code of the State of Ohio, and in accordance with Section 82 of the Charter of the City of Youngstown, and with Section 731.41 of the Revised Code.
