722.04 OTHER REQUIREMENTS FOR REGISTERING.
   Prior to registration, the chief enforcement officer, or his designee shall require an applicant to furnish the following:
   (a)   Bond. A ten thousand dollar ($10,000.00) surety bond, issued by a surety authorized to do business in the State of Ohio, made out to the City of Youngstown, conditioned upon the specialty contractor’s faithful performance of all work entered into or contracted for by such specialty contractor, and upon the specialty contractor’s strict compliance with all provisions and requirements of the Codified Ordinances of the City of Youngstown. This bond shall be used to indemnify the owner of each project that the specialty contractor is performing work on against work not performed to code by the specialty contractor; and
   (b)   Insurance. A certificate of insurance evidencing that the applicant has insurance for bodily injury in at least the amount of $300,000.00, and for property damage in at least the amount of $100,000.00. Each such insurance policy shall carry an endorsement which requires that the City be provided with at least a thirty (30) day written notice in the event that such insurance is to be cancelled or not renewed for any reason; and
   (c)   Workers Compensation. An unexpired copy of the specialty contractor’s Ohio Worker’s Compensation Certificate; and
   (d)   A current Federal Tax Identification Number, or Social Security Number; and
   (e)   Proof that the specialty contractor holds a valid and unexpired license in the respective trade as issued by the State of Ohio pursuant to Chapter 4740 of the Ohio Revised Code, or proof that the specialty contractor has passed an examination in such trade as issued by the City of Youngstown, or proof that the specialty contractor has passed a qualified examination in such trade that is approved by the City’s chief enforcement officer or his designee.
