1709.02 INFORMATION TO BE OBTAINED.

   Any physician who diagnoses or treats, or other person who otherwise has knowledge of a case or suspected case of a sexually transmitted disease shall, in addition to the report required in OAC 3701-3-02 to 3701-3-07 obtain the name, address, age, sex, color and such other information as will permit such person to be located, of all persons associated with such case or suspected case who might reasonably be suspected of harboring or having acquired a sexually transmitted disease, and shall include such information with the case report to the Health Commissioner.
