A petition to revoke a permit shall be filed with the Clerk of the Board by the Enforcement Officer, accompanied by a statement of the grounds or reasons for such action.
Upon the receipt of such a petition, the Clerk of the Board shall set the matter for a public hearing and shall serve the permittee with a notice of the hearing by certified mail, and a copy of such petition, no less than fifteen (15) days prior to such hearing.
