An application for an exhibitor’s license or for an amusement device shall be made to the Clerk of Council on forms provided by him or her. Applications for an exhibitor’s license shall set forth the places of business and residence of the exhibitor. If the exhibitor is a firm, the place of residence of all the partners shall be furnished. If the exhibitor is a corporation, the place of residence of the chief business manager or other controlling executive officer shall be furnished. Such application shall, in addition, state the number of amusement devices for which licenses are requested, the location of the premises and the particular room on the premises in which such amusement devices are to be exhibited. If the Clerk of Council is satisfied that the applicant has complied with all of the provisions of this chapter, and is otherwise eligible to be a licensee, he or she shall issue the license upon payment of the license fees. Licenses for amusement devices shall specify the locations in which they are to be exhibited or maintained. Each license shall be attached to the machine for which it is issued, so as to be in plain view of the persons playing the same.
