Applications for all licenses and permits required by this chapter, shall be made in writing to the village clerk on the application form provided by the clerk. Each application shall state the name and address of the applicant, the purpose for which the permit or license is desired, the location to be used, and the time for which the permit is sought in the case of a temporary permit.
