155.09  LEAVE OF ABSENCE; PROVISION OF MEDICAL BENEFITS.
   (a)    Each full-time employee shall be entitled to request an unpaid leave of absence.  An unpaid leave of absence may be requested in writing to the Mayor.  The request shall state the reason for the leave and the expected duration of the leave.  The Mayor shall have the right to approve, disapprove or modify and approve any leave of absence request.
   No leave of absence which is requested for any reason covered by Windham’s Sick Leave Policy (See Section 155.03) shall be granted until after the employee requesting the leave of absence has utilized all of the sick leave credit standing in that employee’s name.
   
   (b)   A full-time employee shall receive continued health benefits under Section 155.05 during the following periods of time:

Amount of service to Village      benefits will be paid by Village
1-2 years               1 month health care
3-4 years                2 months health care
5 or more years             3 months health care
   Alternatively, if the employee in question would be entitled to receive health care for a more extended period of time pursuant to the Federal Family and Medical Leave Act than this enactment would provide, the employee shall receive the benefits called for under the Federal Family and Medical Leave Act.
   No other employee on unpaid leave of absence shall receive health benefits under Section 155.06.  (Ord. 1418.  Passed 9-12-95.)
