155.06  HEALTH INSURANCE.
   (a)   The Village shall provide each full-time employee with hospitalization and medical insurance in such amounts as shall from time to time be determined by Council.  A full-time employee is defined as being an employee who has been employed by the Village for at least thirty days, and is assigned to work at least forty hours per week.
   (b)   The Village shall provide each part-time employee with hospitalization and medical insurance in such amounts as shall from time to time be determined by Council.  A part-time employee is defined as being an employee who has been employed by the Village for at least thirty (30) days, and is assigned to work an average of twenty-five (25) or more hours per week.
   (c)   The Village Council shall periodically establish Village policy as to the amount of Village contribution toward the premium for employee medical insurance, for both full and for part-time employees. 
   (d)   Employees are required to cooperate in Village applications for group medical insurance and to timely fill out individual applications for medical insurance when requested to do so by the Village Administration. Timely in this instance means completing the application within five (5) business days of the request. In the event an employee fails to timely complete a requested medical application the employee may, at the Village's discretion, be prohibited from participating in the medical benefit offered to Village employees for the next year.
