(a) Application for the billiard and pool room license shall be made to the Director of Finance on forms to be prescribed by him, and shall state the name and address of the applicant, if individuals, and if a firm or partnership, the names, residences and occupations of the manager and each member of the firm or partnership, and if a corporation, the names and residences of its officers and manager, and such other information as the Director may reasonably require.
(b) The application shall also state the premises where such room or place is to be owned, opened or maintained, the name and resident address of the manager or person in charge thereof, whether any of the persons required to be named in the application have been engaged in the operation of such a room or place within five years next preceding the application, and when, where, and how long they have been so engaged.
(c) The Director shall transmit such application for inspection and investigation to the Building Inspector, Fire Chief and Police Chief. The Building Inspector, Fire Chief and Police Chief each shall, after such inspection and investigation as they may deem necessary, make a report with their recommendation for approval or disapproval of such application. There shall be submitted with and as a part of the application, plans showing the pertinent details of the billiard and pool room and the building as required by the Building Inspector. These plans shall be submitted in sufficient copies for distribution by the Director to the Building Inspector, Fire Chief and Police Chief, and all members of the Planning Commission.
(d) If the application and the plans are approved by the Planning Commission, Building Inspector, Fire Chief and Police Chief, the Director shall issue a license on payment of the required fee.
(e) The application for license shall be rejected if the report submitted thereon shows that any person named in the application is not of good moral character or that the billiard and pool room sought to be licensed does not comply with the zoning, building, health and fire regulations applicable thereto. No billiard and pool room license application shall be approved and no billiard and pool room license issued until the applicant has furnished evidence of single limit public liability insurance for bodily injury and property damage in the minimum amount of three hundred thousand dollars ($300,000). If the sale of alcohol is permitted on the premises, no billiard and pool room license application shall be approved and no billiard and pool license issued until the applicant has furnished evidence of single limit liquor liability insurance for bodily injury and property damage in the minimum amount of three hundred thousand dollars ($300,000).
