Applications for licenses to be issued pursuant to the provisions of this chapter shall be made to the Director of Finance upon such forms as shall be prepared by such Director. The application shall be made by the owner or proprietor of the business or place at which the drug paraphernalia are to be sold. The application shall state the name of the owner of the place of business, the address of the place at which the business is being conducted, the residence address of the owner or manager of such business and shall contain a representation that neither the applicant nor any employee of the applicant authorized to sell such items has ever been convicted of a drug related offense.
Any person, firm or corporation to which a license is issued under this chapter shall pay an annual fee of one hundred fifty dollars ($150.00). All licenses granted hereunder shall expire on July I annually. The license shall be displayed in a conspicuous place in the establishment for which it is issued. No transfer of any license shall be permitted.
