Application for the permit required by this chapter shall be upon a form provided by the Director of Public Safety. The application shall include the following information:
(a) The name and address of the owner of the property where the boarding or rooming house is conducted or is to be conducted.
(b) The location at which the boarding or rooming house is conducted, the location and number of rooms to be devoted to such use and a floor plan of the home or building with which such application is concerned.
(c) The name, age, employment, marital status and residence address of the proprietor or owner of the boarding or rooming house, and the name, age, employment, marital status and residence address of the tenants or other occupants of the home or building with which such application is concerned.
(d) The date upon which such boarding or rooming house commenced or is to commence.
(e) Where the applicant is other than an individual, the name, address and telephone number of an individual charged with the management of the boarding or rooming house.
(f) Such other relevant information as the Director of Public Safety, in his discretion, shall require.
