751.04 ISSUANCE OF CERTIFICATE.
   The City Building Inspector shall issue a certificate of registration to each applying contractor when each of the following conditions have been met:
   (a)   The applying contractor has completed the application form to the satisfaction of the Building Inspector.
   (b)   The applying contractor has furnished evidence of liability insurance for bodily injury in the minimum amount of one hundred thousand dollars ($100,000) per person, three hundred thousand dollars ($300,000) per occurrence and for property damage in the minimum amount of one hundred thousand dollars ($100,000), or a combined single limit of liability in the amount of three hundred thousand dollars ($300,000). Evidence of the above insurance shall be in the form of a certificate of insurance which shall certify the following:
      (1)   The insurance coverage is written on a comprehensive general contract with evidence of motor vehicle liability insurance in the above limits;
      (2)   Products and completed operations is in force;
      (3)   That any “XCLT' exclusions are deleted;
      (4)   The City of Willowick is added as an additional insured;
      (5)   The City of Willowick is provided thirty days advance notice of cancellation, termination or partial change in coverage or limits of liability.

   (c)   The applying contractor has furnished and filed with the Building Inspector a bond in the sum of fifteen thousand dollars ($15,000) to be on such form as approved by the Director of Law guaranteeing full and faithful compliance with all the provisions of the City Building Code, indemnifying and holding the City free and harmless all as provided therein.

   (d)   The applying contractor has tendered the certificate of registration fee as specified in Section 751.05.

   (e)   The applying contractor has tendered proof that it has registered with the Regional Income Tax Agency.
