(a) Application for a certificate of registration shall be upon a form provided by the City Building Inspector. The application form shall include space for the true name, current address and telephone number of the contractor applying and designate the form of business organization, whether corporation, partnership, individual or other organization. When the applicant is other than an individual, he shall state the name, address and telephone number of an individual charged with the management of the work of the business organization to be performed within the City. The application shall contain such other information as the Building Inspector deems pertinent.
(b) Any contractor who makes an application for registration as set forth in subsection (a) above, who is required to be certified by the State of Ohio pursuant to the provisions of the 2006 Residential Code of Ohio, shall not be permitted to register until such time as the contractor places on file with the City Building Inspector a true and accurate copy of the applicable state certification.
(c) Any contractor who makes an application for registration as set forth in subsection (a) above, shall not be permitted to register until the contractor produces a true and accurate copy of the applicable certificate which evidences that the contractor has in place Workers’ Compensation Insurance.
