Any person desiring a certificate of necessity shall file with the Director of Public Safety a written application therefor, on a form to be provided by the City. The application shall be in affidavit form and contain the following information:
(a) The full name, age and residence of the applicant and if the applicant is a partnership, the full name, age and residence of each of the partners, and if a corporation, the full name of the corporation together with the full name of its principal officers.
(b) The name under which the applicant will do business.
(c) The type of vehicle(s) which is proposed to be used in service, with information as to age, carrying capacity and motor power of the same; the color scheme, local symbols to be used together with all other lettering or marks proposed to be used by the applicant on the vehicle.
(d) The number of vehicles proposed to be used and the rate of fare to be charged and the method of charging the same.
(e) A certificate of insurance executed by a duly licensed representative of an insurance company licensed to sell motor vehicle insurance in the State in conformity with all requirements contained herein.
(f) The Director may, as he may desire, require such other information as to the applicant's financial or other fitness to safely conduct the operation of a public vehicle service in the City.
(g) The application shall be accompanied by a fee of one hundred dollars ($100.00) to cover the expense of Council and the City in investigating the applicant, serving notice of public hearing and the granting or refusal of the certificate.
